Your first appointment will include an evaluation and possible testing. In order to get accurate results, some medications need to be stopped prior to the visit. PLEASE CLICK HERE FOR MEDICATION INFORMATION.

Please arrive 15 minutes early. If you arrive more than 10 minutes after your scheduled time, you may need to be rescheduled and incur a fee.
We send out several reminders for your appointment. If you are unable to keep it, we require minimum of 24 hours’ notice to avoid a nonrefundable $75 cancellation fee.
A parent or legal guardian must accompany their child for the first appointment.
We ask that you please be mindful of others that are coming to our practice for care and not smoke or wear perfume/cologne prior to your visit.
Food of any kind is not allowed in the waiting area.
Testing can be expensive. Financial counseling is available by contact the billing department. Your provider will only order what is appropriate for your care.
